Attach a feedback form with ajax to the current html

There are ready-made feedback form in html
you must attach this form to the code (taken from github, suspended) sending data to mail.
for example, this (github.com/Gix075/ajax-co... or the other
Requirements to the form:
- sending via ajax
- attaching files
- logging of sent requests
