Feedback form for Joomla 1.5

On the ancient site of managements Joomla 1.5 you want to add a feedback form from a template (bottom) design on all the pages of topics/materials. You can use the plugin.
Form fields – Name, phone Number, E-mail, Commenatry

The form should emerge by clicking the button "place order" and go to e-mail, which we will specify in the settings
