Form to send emails with a landing page

Hello!

Need to make a working form to send mails to email on the landing page

I. e., Lida 5 forms, now they dont work...

The first three forms to order goods with a different name - the letter should be specified what form the order came and there should be fields for:

Behalf of the client

phone client

And client messages.

Fourth form - subscribe to site news should come a letter saying that the person just subscribed to the newsletter (Name, e-mail).

Fifth form - by person asked the admin of the site in any form (fields left as is).
