Make/adjust form on the website minor edits

good Day. There is a website (nbcon.ru/mnp/), now move to a separate domain. Need:

1. To get to work the first main form (on the video), before there were other fields. Need to transfer data from a form and check fields of the contacts.

2. To make a popup form that you can bind to any button (in this situation, the big red "request a manipulator" in the third block). Now there is no form, you need to pop up by clicking on the button, well, of course, transmit data/checked zapalenie contact fields. 5. Explain how to copy the popup-form and quick order for the rest of the pages (well, pull up everywhere here's a script from CDN, and paste this script and give the button a class), something like that :)
